2011-12 Sunday School Registration Form	Sunday School is for 3 years old through 5th grade. You will have to register each child separately. To help cover the cost of Sunday School materials a donation is appreciated. (Suggested $15/child or $35/family). If you have 3 or more children, you will need to register at church for the family rate of $35. Everyone is welcome regardless of money!Please note, your child's photograph may be taken for projects within their Sunday School Class or possibly a video to be shown during worship.
MOPS 2011-12 Registration Form	Our yearly fee is $75 ($25 MOPS International Fee, $25 first semester, $25 second semester). You may pay the total now or pay per semester with check or cash. However, filling out this registration form does not guarantee your enrollment in MOPS. Availability will be determined when MOPPETS workers are secured.
